In the chaotic days following Hurricane Katrina, rescue boats floated past rooftops while command centers hundreds of miles away got stuck in red tape. Ambulances sat idle. Supplies were stuck in warehouses. FEMA officials sifted through contingency plans that no longer matched the realities on the ground. With phones down and roads submerged, responders on scene waited for approvals that never came. It wasn’t a lack of planning that failed New Orleans — it was the belief the plan was still in control.
One of the core challenges in high-stakes environments is psychological, not procedural. Any emergency can trigger this, but large-scale disasters like Katrina expose a critical danger: the illusion that detailed plans equal true preparedness. In reality, rigid systems often break down in chaos. The push to “follow procedure” frequently delays action, while teams that trusted their instincts and adapted on the fly were the ones who made a real difference. The more we cling to the plan, the more we risk freezing up when it no longer fits.
There are well known behavioral theory reasons for this. Economics and informatics both show how centralized systems struggle with real-time complexity. When information is incomplete, delayed, or constantly shifting — as it is in a disaster — top-down plans falter. Decentralized, adaptive decision-making tends to outperform rigid control in dynamic environments. So, how do we strike the right balance of prepared but flexible, structured but adaptable? To answer that, let’s look at the common underlying issues that cause systems to fail and explore how to address them.
The Psychology of Control and Its Pitfalls
People know they need to adapt in a crisis, but even when we understand the need for adaptability, our brains crave structure and predictability. This natural impulse becomes especially dangerous under pressure. Behavioral economists call it the illusion of control. This is our tendency to overestimate how much influence we have in uncertain situations. The more detailed the plan, the more we believe we’re in command. But when reality veers off-script — as it often does on a fireground — a rigid mental model can backfire. A sudden wind shift, equipment failure, or flashover can unravel even the best-laid plan. When we’re too anchored to the original plan, we hesitate. Instead of adapting, we try to force reality to fit the plan — often wasting precious time.
To counter the illusion of control, we need to shift from rigid execution to dynamic sensemaking. Sensemaking is the process of interpreting unfolding events, recognizing key changes, and adapting in real time. When a situation breaks from the script, we experience what’s known as a routine disruption; our brain recognizes the mental model no longer fits. Ideally, we adjust. But when we are so committed to a detailed plan, the mental effort required to rebuild our internal narrative can be overwhelming. Informatics researchers call this a model violation — a breakdown between the system’s representation (the plan) and the first responder’s real-time perception (the scene). If our tools and processes — like SOPs, checklists, or leadership structure don’t support flexible thinking, they can actually deepen the paralysis. This paralysis can be counted or identified in training or after-action reviews by looking for things such as long pauses before action, repeated attempts to reapply outdated plans, or delayed communication during unexpected changes.
Another challenge in dynamic environments is the signal-to-noise problem. On a busy scene, it can be difficult to distinguish critical information (signal) from background chatter and irrelevant data (noise). In high-pressure situations, information often floods in from multiple sources: radios, sensors, reports, team members. Not all of it is accurate, timely, or useful. The brain, already under stress, can become overloaded, causing us to fixate on the wrong details or miss key changes entirely. This cognitive noise can drown out the cues that matter most. Effective sensemaking depends on filtering out the noise, recognizing meaningful patterns, and surfacing the right information at the right time. Systems that don’t prioritize clarity and relevance only add to the confusion. Agencies can proactively address this by being aware of this and doing things such as counting the number of conflicting reports or repeated radio messages. Even counting these during a practice drill can give your team a good baseline level.
Cognitive Overload and Decision Fatigue
Beyond information overload, there is also the issue of decision fatigue. This is the mental exhaustion caused by making too many decisions in rapid succession. As cognitive load increases (the number and complexity of choices we must make), our ability to separate signal from noise declines. The brain tires, and each new decision demands more effort. Over time, this wears down our precision. We may overreact to noise, miss critical signals, or become paralyzed by indecision. Even small, routine choices can start to feel overwhelming. In chaotic environments, this often leads people to default to rigid procedures, habitual responses, or complete inaction. The combination of noise and fatigue doesn’t just slow decision-making, it can cost lives.
Under pressure, decision fatigue can distort priorities, impair judgment, and magnify errors. To counter this, organizations and individuals should actively monitor behavioral signals that indicate overload. These include increased reliance on default options, abrupt or irrational shifts in decisions, escalating emotional responses, and avoidance of even basic choices. How this looks in every person is different, and we are particularly bad at recognizing it in ourselves, but some common signs are: team members avoiding eye contact, delaying decisions, or repeating commands unnecessarily. If possible implement rotating decision-making duties or mandated “mental reset” breaks after intense periods.
Preparedness and adaptability are not opposing forces. Firefighters clearly need to excel at both. A well-designed plan should not be a script that locks people into rigid actions, but a scaffold that supports informed improvisation. Preparedness is about giving teams a starting point, shared language, and a map. Adaptability is what lets them redraw that map when the terrain shifts. The best systems recognize this balance and build it in by design. That means preparing first responders to think as well as act. It means creating plans that assume disruption, tools that support decision-making in motion, and leadership that trusts distributed judgment in the fog of chaos. The world is unpredictable. We don’t need to chase perfect plans — we need people who can read the moment, adapt with clarity, and move forward anyway. In the end, resilience is not about following the plan. It’s about staying ready when the plan no longer applies.
